First Line - Your Name as you would like it to read.
This can be your first and last name or your first initial and last name. Please indicate Mr or Ms as it's not always easy to tell from a name!
Second Line - The location where you sleep (now, or when you were 11 and got your letter!). The Second Bedroom to the Right, or The Room Under the Tree, etc.
Third Line - A Street. Again, this can be now or when you were 11, or something more whimsical, such as the Lane by the Lake.
Fourth Line - City
Fifth Line - State, Province, Country or whatever you feel fits best.
